This enables us to establish goals and a plan of care,  provide one point of contact for care, and advocate to support your outcomes for our plan of care.

Placement Support 

Our Care Management services empowers our clients by advocating for their needs, ensuring they receive the resources available for placement. Aging in place is the optimal desire for most clients, but what if that is not possible? We assist families in reviewing all options, based on a budget and expectation for the future of our clients. We will assess the needs, advantage and disadvantages of all options, tour facilities to identify pros and cons with our clients. We do not have a financial relationship with any given home or facility and therefore we work for our clients and their families. We can identify increased costs with the aging process because we do not have a relationship with anyone except our clients. Many placement services have a financial relationship and contracts with facilities and only provide placement for those facilities/homes. The options are endless in Orange & LA County. Without a financial relationship with placement options, we can ensure a professional opinion and guidance with our clients as the forefront of our care.

Advocacy Support provides clients with a voice in their care decisions.  We advocate with housing options, know about negotiating prices and ensure all services are provided within the facility/home.

Aging In Place

Aging in place is the preferred method of housing for the aging population. We can help support several options for this to take place safely and effectively.  We help find the most cost effective way to allow this option for our clients where possible.

Chronic Care Management

Care management service to assist in preventative health. Our goal is to keep clients out of the hospital. The behind the scenes care that happens in between provider visits.

Memory Care Specialists

We are familiar and dedicated to dementia, decline and the natural aging process. Our care managers can assist families with memory care options, facilities, and help decrease frustrations.

  • Manage facilities and staff with guidelines that families prefer. Many memory care facilities have revenue based care, and we can support all options for our clients. We are not revenue based
